The goal of this survey is to help the Centers for Medicare & Medicaid Services (CMS) better understand the data submissions experiences of participants in the Quality Payment Program (QPP). Your responses will help CMS identify areas for improvement and enhance future submissions experiences for QPP participants.

Results will be reported in such a way that no single individual, group, or entity can be identified. Your response is voluntary.
